GENERAL INFORMATION
1. The Church reserves the privilege of using space already leased
on a regular basis should a special need arise. We will give reasonable
notice.
2. Rental charges include the use of existing chairs
and tables only. Please contact Julie Hudak, Business Manager, at
the Church office for further information.
3. Kitchen rental fees include the use of all existing
equipment, contents, etc. NO STOVETOP COOKING THAT CREATES GREASE
LADEN VAPOURS. No equipment to be removed from the Church property.
ALL FACILITIES AND EQUIPMENT MUST BE LEFT IN A CLEAN AND TIDY MANNER.
4. Any additional equipment or furnishings required
must be procured, installed and removed by the user. Contact must
be made with the Business Manager before using additional electrical
equipment to ensure adequate power supply.
5. The Church building must be vacated by 10:30
p.m. on weekdays and 6:00 p.m. on Saturdays.
6. The Church must be notified if alcoholic beverages
are to be consumed and the renter is responsible for obtaining the
LLBO permit, which must be brought into the Church office the day
before the event.
7. The Lessee must provide their own liability insurance
or sign a "Hold Harmless" form on the reverse side releasing
the lessor from responsibility for any personal injury which may
be sustained by the lessee or any person on the premises pertaining
to the rental function.
8. The Lessee must familiarize themselves with the
floor plans of the church as shown on the reverse including all
fire escapes. The Lessee must observe our No Smoking policy on the
premises.

I/We, the undersigned, on behalf of________________________________
both during and following the term of this agreement, undertake
to indemnify and save harmless the Synod of the Diocese of Niagara
and the Parish of St. Jude's Church from and against any and all
loss, cost, damages, expense and liability (statutory and common
law) in connection with the injury or death of any person or any
property or other damage sustained by The Synod of the Diocese of
Niagara, or St. Jude's Church, its directors, officers, employees,
agent or volunteers, which may arise out of my/our use of the Church
property.
I/We also undertake to conduct my/our activities
in a safe and careful manner and I/we assume full responsibility
for the conduct of my/our students/employees/members/volunteers
and for their safety, and for any damage caused to the church property.
I acknowledge the regulation of "NO COOKING IN THE KITCHEN
OF ANYTHING THAT CREATES GREASE LADEN VAPOURS" and have familiarized
ourselves with all fire escape routes and extinguishers (see pictures
on site for detail).
